A 14-year-old Rio Vista girl was killed in a single-vehicle crash Monday morning.

Rescue workers responded about 2:40 a.m. Monday to the intersection of county roads 1204 and 1102.

According to reports, a vehicle with two occupants lost control and ended in a wooded area.

One of the occupants, identified by the Tarrant County Medical Examiner as Callie Nicole Ivy, 14, was pronounced dead at the scene.

The second occupant was transported to a nearby hospital in unknown condition.

The crash is being investigated by the Department of Public Safety.

Multiple agencies responded to a fatal wreck near the Chisholm Trail Parkway on Saturday. About 11 a.m., rescue workers responded to the 5300 block of County Road 915 in Joshua where a vehicle crashed into a tree.

According to a press release from DPS, a preliminary investigation shows that a 2012 Ford Mustang was traveling west on County Road 915 when, for an as of yet unknown reason, the mustang drove off the north side of the roadway and struck a tree. This caused the Mustang to spin, ejecting three of the four occupants before it came to rest.

The driver, identified by DPS as Shawn Johnson, 47, of Red Oak, was pinned in the vehicle and had to be extricated by emergency personnel. He was transported by air to John Pete Smith Hospital in Fort Worth for serious injuries.

Two of the passengers, identified by DPS as Amy Wright, 33, of Conroe and Shawandasee Begley, 46, of Cleburne, were both transported by air to Texas Health Harris Methodist Hospital in Fort Worth for serious injuries.

A third passenger, identified by DPS as Mathew Warner, 32, of Crowley, was pronounced dead at the scene.

The cause of the crash remains under investigation by DPS.

A woman was killed in a head-on collision in Burleson later that day.

Rescue workers responded to the scene about 2:20 p.m. in the 6800 block of FM 1902.

The driver, identified by the Tarrant County Medical Examiner as Charity Lynn Herrin, 26, was ejected from her vehicle and was pronounced dead at the scene.

A child passenger was airlifted to Cook Children’s Medical Center in Fort Worth for serious injuries.

The cause of the crash is still being investigated.
